DATES FOR 2010 SUMMER MIDDLE SCHOOL REGISTRATION  HAVE BEEN SET: MONDAY AUGUST 23 FOR 8TH GRADE, TUESDAY AUGUST 24 FOR 7TH GRADE, AND WEDNESDAY AUGUST 25 FOR 6TH GRADE. PACKETS WILL BE MAILED OUT IN MID JULY 2010.

Dear Middle School Parents and Guardians,
 
In an effort to ensure a smooth and efficient opening of the 2010-2011school-year, South Pasadena Middle School will be holding our annual Registration on August 23rd for 8th graders, August 24th for 7th graders, and August 25th for 6th graders. You should  receive your appointment time to register your student in a mid July mailing from the middle school.  The time breakdowns are as follows:
 
• Monday, August 23 : 8th grade
o Students with last name A-G: 8:30 AM – 10:00 AM
o Students with last name H-M: 10:00 AM – 11:30 AM
o Students with last name N-U : 1:00 PM – 2:15 PM
o Students with last name V-Z : 2:15 PM – 3:30 PM
 
• Tuesday, August 24: 7th Grade
o Students with last name A-G: 8:30 AM – 10:00 AM
o Students with last name H-M: 10:00 AM – 11:30 AM
o Students with last name N-U : 1:00 PM – 2:15 PM
o Students with last name V-Z : 2:15 PM – 3:30 PM
 
• Wednesday, August 25: 6th Grade
o Students with last name A-G: 8:30 AM – 10:00 AM
o Students with last name H-M: 10:00 AM – 11:30 AM
o Students with last name N-U : 1:00 PM – 2:15 PM
o Students with last name V-Z : 2:15 PM – 3:30 PM
 
 
 Packets will be mailed this year. If you do not receive a packet, you may pick one up at the SPMS office after August 18, 2010.
   
For those of you that have registered before, you know that there are many district forms that must be read. District-wide we have used more than 45,000 pieces of paper each fall to communicate all necessary information to you!
 
Again this year: In order to save paper we ask you to view all of the registration documents by clicking the links below and signing a "District Acknowledgment Form" to verify that you have read the documents.  New this year: The district will not be requiring proof of residency for currently enrolled students at registration. You will only need to bring in three current proofs of residence if you have recently moved to a South Pasadena address which is different from the address of record in the school's database. South Pasadena schools begin on Monday August 30, 2010.
 
The first link is for a "District Acknowledgment Form" for you to sign and confirm that you have opened and read all the registration documents.  We ask that you sign and return this form during registration in August.
 
To be prepared for registration you should:
1. Update and bring in the "Student Demographic" form enclosed in your registration packet.
2. Read the on line Registration Documents below (the links should be updated and valid by July 12, 2010).
3. Print and Sign the "District Acknowledgment Form".
4. Report to school on August 23, 24, or 25 for registration.
5. Please note that all links below will not be updated until JULY 12, 2010. Please do not download/print them until this time.

The following documents will also be given to you at registration but we have included them here for you viewing.
• Student Accident & Health Insurance Brochure (provided in your registration Packet - not available online)
• Emergency/Information Card (provided in your Registration Packet - not available online)

 Students who do not register will not be able to be placed in class. If you have additional questions, please contact the school office after August 17, 2010.

**Residency Verification Documents for New Registrations, or verification of a new address within South Pasadena:
 
Just a reminder, when registering a new student, bring one of the documents from EACH category listed below:
 
Category A
Deed of Trust
Mortgage Payment Receipt – dated within the past month
Rental/Lease Agreement, listing all names of occupants (including children), and the name, address, telephone number and signature of landlord/management company.
Property Tax Bill – dated within the past year
Closing Escrow Papers
 
Category B
Utility Bill (Example: Southern California Edison, The Gas Company, Water Bill) – dated within the past month
Time Warner Cable Bill – dated within the past month
Telephone/Cell Phone Bill – dated within the past month
 
Category C
Current Car Registration – dated within the past year
Car Insurance Statement – dated within the past 6 months to 1 year
Health Insurance Statement – dated within the past month
Bank Statement (or signed letter on bank letterhead with the South Pasadena address) – dated within the past month
Pay Stub showing current South Pasadena address – dated within the past month
Current Federal or State Tax Forms – dated within the past year

** For a newly established residence, for verification from Category B, a signed, original receipt or statement on letterhead from the utility company will satisfy the requirement, provided the name of the parent/guardian and South Pasadena address where service will be established are both listed.
